Student Feedback questionnaire with core questions which assesses the lecturer, module assessment, practicals and tutorials (where applicable). As teaching and learning is a critical component in a number of performance and teaching development and support initiatives at the University, a university wide generic questionnaire was designed.
The Postgraduate supervision questionnaire for postgraduate students (Honours to PhD) for the research supervision. This questionnaire was designed to gain feedback on research supervision and acts as one of the measures that should be considered to assess the quality of research supervision provided to student
Two Student Feedback Questionnaires on the professional practice module (PPM) and professional practice supervisors (PPS). These questionnaires allow for modules with a professional service component to be evaluated by students
Electronic: The Student Feedback and the Professional practice questionnaires can be administered via Moodle. You can consult the guide below on how to request for a QPA questionnaire to be added onto your Moodle course (click here to request a QPA questionnaire). For any assistance on the Moodle site please navigate to the Moodle Learn site click on the Resources page, click on Moodle Support and navigate to QPA Student Feedback Evaluations. Should you require further assistance, please send an email to learn@ukzn.ac.za with the Subject: QPA Evaluation assistance.
Paper base: The QPA Student Feedback Questionnaire for module, lecturer, practical and tutorial feedback (available below) which consists of compulsory questions. Note that the paper base evaluation submissions must be accompanied by the Student Evaluation Feedback Cover Sheet (click link to download).
